Traffic Accidents: How to Settle a Minor Traffic Accident With a Pedestrian

My question involves an injury that occurred in the state of: California

Yesterday I accidentally hit a pedestrian. I was going pretty slow at the time and there was no serious injury. It was a minor and her dad called 911. The cop that came said he was going to fill out a police report although normally would not because of how minor the accident was he was doing it because she was a minor. The cop also called the fire department to look her over because she was a minor. The fire department found no serious harm and said she would have a bruise for a couple of days. The father agreed to settle for $375. The cop told me I should just ask him to sign a paper saying he accepted the payment and that would be good enough. I asked if I should have it notarized and he said it was not necessary. Is he right or should I have it notarized? Also, the cop said I had to report it to my insurance company even if I do not file a claim. But when I called my insurance company they said they do not accept reports they only file claims. So now I am confused what I should do because the cop said the insurance company would get a copy of the police report so now I am worried I will get in trouble and I do not know what to do. Also, I included a copy below of the paper I am going to ask the man to sign. Should I say or include anything else in the paper?

On February 17, 2017 ******* hit ********, 17 year old daughter of ********. ******** agrees to accept $375 and will not sue, press charges, or file a claim against ******** over the accident. ********* also agrees not to ask ********* for any other payment in any form regarding the accident.
