J Visas: Special Case: Marriage on an Unfulfilled J-1 Visa

The situation: My girlfriend of three years is in the first year of her J-1 visa which has an alotted timeframe of 4 years for her field of medical training, her visa includes the 2 year return of service requirement. She is not continuing in her current position after June and will be taking one year off from residency. We are getting married in a few weeks. I understand that she can't apply for a green card until she either completes her return of service or is granted a waiver for a specific reason (I have read through some other great threads on this site-thanks!!).

Questions:
1. Will she have to return to her home country during this year off given that she does not have an official residency?
2. Are there other options for staying in the US during this year?
3. Will she have any problems related to her visa status applying for a position after a year off?
4. When she reapplies for positions after her year off will she still require sponsorship of a visa from an employer even if we are married at that point?
5. Is there any way that she can terminate her J-1 status and receive a green card to be used from this point forward? (I know this probably sounds quite utopian but I just wanted to ask).
