My question involves public health law in the State of: Connecticut
I have questions concerning CT Death Certificate (VS-4 Rev 1/04)
1. The Pronouncer completed date, time and name/title blocks but didn't sign or date the signature block. Also this person is a Physician Assistant
2. This same PA.C has filled out the Certifier section except checking the Certifying practitioner or Pronouncing & Certifying practitioner blocks and he didn't complete the mailing address portion.
This document has already been received and entered into the official record by the Vital Statistics section.
Isn't a Medical Doctor signature and/or review required in a death that occurred in a hospital or Am I wrong that this PA.C is already an attending and has the full authority of a MD in Connecticut?
