Other Violations: Getting a Ticket in the Mail but Never Got Pulled Over

My question involves a traffic ticket from the state of: NY

My husband got a letter in the mail today from the DMV, saying that his licence will be suspended on 2/19/16 because he failed to answer a summons. The letter states that on 9/25/15 that he was issued a ticket for operating a motor vehicle with a portable electronic device. The problem is, he was never pulled over, and this is the first time since that date (nearly 4 months ago) that we have received any type of letter in the mail regarding this situation. We aren't really sure what to do at this point, the ticket they are trying to claim comes with a hefty fine and 5 points on your licence in New York State. Are the police even allowed to issue you tickets without pulling you over? Any advice or suggestions?
