Establishing Paternity: How to Correct a Birth Certificate After a Paternity Determination

My question involves paternity law for the State of: Pennsylvania

My husband has a child with another woman whom was married (unknowingly) to him at the time. We have a court ordered paternity test, support order and a legal name change however she still has her ex husband listed on the Birth Certificate. We want to get it changed to reflect not only my husbands name as the father but also to have my step sons last name changed to match his legal name.

I am getting worried because this has been brought up several times and after the mother saying she will get things started and taken care of nothing has happened. It will be difficult for him in the future to do certain things and get a driver's license, passport etc when his name does not match his birth certificate or social security card.

Can someone please direct me in how we can start this process as it doesn't follow the normal "unwed mother" in PA rule and I can't seem to find any info on it. Thank you.
