Obamacare agencies have messed up my information (address & birthdate) and will not fix it & will not send me my 1095 form to file my taxes (I had to delay my taxes) I have been requesting this info since March, to find out they have messed up my info! they claim to have fixed the problems, but will not send info concerning anything they have told me over the phone. Everyone I have talked to has said, we will get that right out to you or its in the mail, I'm tired of the excuses. Please advise me on what I can do to get this mess fixed. Thank you
