Court Proceedings: to Get Deceased Husband on Birth Certificate

My question involves paternity law for the State of: Ohio

My husband passed away a month ago. We have 3 children together. For the first two children, we were not married. He did not have his ID at the hospital when it was time to sign for the birth certificate, so his name did not get added to our two daughters at the time of birth. For our oldest daughter, we filled out the paternity affidavit, mailed it in, and he was added. We never got around to doing this for our second daughter, so his name is not on the certificate. We got married when she was 9 months old, and stayed married and together until he died last month. I now want his name added to her birth certificate, for personal reason and for Social Security (mainly concerned with personal). I have started DNA testing (there is no question, but know this needs to be done.) and am just waiting on the results to proceed with the next step. That is what I am unclear of. What is the next step? What needs to be done in court? I am being told to get a lawyer, but would rather not if it is possible. We were always together, there was never a question of whether or not he was her father. It was just that he did not have his ID with him at the hospital, and we were lazy and did not mail in the form. Any help would be greatly appreciated!
