Traffic Accidents: What Medical Bills Are Used to Determine Special Damages

My question involves an injury that occurred in the state of: GA

I've done a lot of research in how settlements work, how attorneys are paid, etc. My accident is a fairly typical case with treatment about to finish and some lingering effects likely.

I understand some lawyers and most insurance companies use a "multiplier" of specials to determine general damages. When using medical bills as part of the specials, which version is used? Is it the provider's bill? Or is it the health insurance's agreed rate?

For example, suppose I have an MRI with a bill of $3500. My health insurance agreed network rate is $1200 (these are not actual). Which one is used?

The reason I ask is that if a settlement is based on a multiplier (I know that many are not, but many settlement offers start there), then it would seem to me that the bigger general damages award would go to someone who does not have health insurance. That, or someone whose rates are different would get a different amount in general damages, which does not seem equitable. I have not been able to find this addressed anywhere in my research.
