My question involves insurance law for the state of: TX. Wife is being treated by a cancer facility. The facility made arrangements for a biopsy at the hospital next door which they work closely with. The approval dates were listed as starting Sept 14. Then they scheduled the biopsy for a few days earlier than Sept 14. BCBS declined processing the claim due to not being approved at that date. When I called BCBS the person I spoke with suggested the provider call and correct the dates. I have been trying unsuccessfully for several days to get the provider to do that. Can the dates be changed? Is the facility liable for the cost of the error? Or might I have to pay. Hospital is wanting $15000 but an in-network adjustment would be more like $5000.
