Custody and Visitation Issues: How to File a Motion for

My question involves a child custody case from the State of: Oregon, Washington

Case: Divorced
Kids: Minor, 2
Custody: joint custody - 50/50

Visitation was clearly stipulated in our divorce papers, and signed by the judge.

Situation:
Spouse refuses visitation.

I have no other choice and claim my visitation rights.
I know that there are so called MOTION's - but I don't know what specific Motion I have to file for.

Where can I find the Motion and steps I have to follow through in order to get this done?
