Health Insurance: Subrogation Questionnaire

My question involves insurance law for the state of: California

I had a car accident almost 3 years ago, and settled with the other driver about 6 month ago. I sent all my medical bills to my lawyer and most of them were taken care of. Today I received a subrogation questionnaire asking if my Doctor visit 8 month ago was due to issues caused by an accident and wants me to provide info on any settlement and/or the other party who caused the accident. I assume this is so my health insurance company can get some money back from him. I went back to my settlement documents and saw this particular visit to my Doctor showed up in the settlement but has no $ amount - most likely I didn't receive the bill at that time. This doctor visit also had some additional lab works that were completed.

My questions are

1. Can my health insurance company get my settlement money I received 6 month ago?
2. Do I need to respond to this kind of questionnaire? If so, how should I respond?
3. What if I am no longer with this health insurance provider or changed job?
