Dismissal: Invoking the Rules

My question involves court procedures for the state of: Florida.
I want to file a motion in small claims court, for dismissal of the pre-trial hearing, mediation, and the trial.
Do I need a special form (or format) for the motion? Or can I just ask the judge (verbally)?
