Traffic Accidents: No Fault

My question involves an injury that occurred in the state of: Florida.

Hello,

Back on 09/23/2016 my fiancé was driving my car and my car was hit. (no my fiancé was not on my insurance) However, the guy hit my car and was the at fault driver he even said "sorry I didn't see you". The police did come and there was no damage/Injuries. However recently this month of March 2017. I received a letter in the mail from Wilber law firm stating that State Farm filed a claim and that I owed for $1500 worth of damages. There was no police report filed but there was a "Accident Investigation Report Withdrawal Form" signed by both drivers which states that 1) There was no fatality or bodily injury. 2) Criminal Traffic Violation and 3) Total Damage is less than $500.00. I do have the form and I called the law firm as of yesterday and they stated that State farm did an investigation and that he was that at fault driver but that is not the case at all. I advised I will be sending in this form to them but I'm not sure if I actually have to handle it through them or if I can speak with someone else. They were very rude and all they wanted to do was get a payment out of me. I don't want to send them this form and them not actually look into it. Also, how can they investigate it completely when there is no police report filed... that states what happened and who the at fault driver is . I don't know what to do from here . HELP PLEASE.
