Social Security Issues: Rep Payee - What Address Do I Use on the Checking Account

I'm a new Rep Payee for a relative. I know the account has to be owned by the relative, needs to be clearing showing that the account is the relative's name by me Rep Payee and that he can have no direct access. Here's what they don't tell you. What address do I use? 1st - I have to write the checks so vendors will check to see if the checks match my id address. 2nd - The statements should come to me and most banks I've talked with here (not the greatest banks in this small community I'll grant you) don't seem to be able to have a residence (of the relative) address on the checks and yet mail me the statements. I don't necessarily want my address on it (except the fact that if the statements go to the relatives address they tend to disappear) and it would help the relative get a state id if the checks had his address.

just don't know what I'm supposed to do and the little booklet doesn't touch this one at all!
