Collection and Enforcement: How to Domesticate an Out-of-State Small Claims Judgment

My question involves court procedures for the states of PA & IL:

I'm looking to domesticate a judgment In Illinois that I received in PA. I have the "Foreiogn Judgment Registration Cover Sheet", the "Notice of Filing to Register a Foreign Judgment", and an authenticated copy of the judgment. I think all that I'm missing is the affidavit, which leads m,e to my question.

Is there a specific form for the affidavit, or will any general affidavit with the pertinent information work?
