MY DAUGHTER'S MOM WAS KILLED BEFORE WE COULD GET MY NAME ON MY CHILD'S BIRTH CERTIFICATE. I HAVE TAKEN THE PATERNITY TEST AND SHE IS PROVEN TO BE MY DAUGHTER AND I AM RECEIVING HER SOCIAL SECURITY CHECKS EVERY MONTH. I WANT TO CHANGE HER LAST NAME TO MINE AND HAVE MY NAME ON BIRTH CERTIFICATE AND CANNOT AFFORD AN ATTORNEY. WHAT DO I DO?
