Deeds - Quitclaim: How Do I File a Survivorship Deed to Transfer Property to Me Solely

My question involves real estate located in the State of: Georgia. When my [late] husband and I purchased our home in 2003, the Deed stated "Joint Tenants with Rights of Survivorship." It is 9 years since his death and I would now like the Deed in my current name. The Clerk of the Court would not accept the Quit Claim Deed I presented and would not tell me the name of the documents I need to get the transfer. I would like to know what I need to present to the Clerk in order to get the documents filed.
